Retroperitoneal Rerforation of the Duodenum by Blunt Trauma Report of 3 Cases with Review of Literature

Abstract

|
Retroperitoneal perforation of the duodenum by blunt abdominal injury is oftentimes a challenging problem to the surgeons, mainly because of difficulty in establishment of early diagnosis, hence resulting in high morbidity and mortality. The duodenum being a fixed and retroperitoneal organ except for the first portion is vulnerable to external violance, and incidence of associated injury to the adjacent structures is relatively high.
Treatment of choice in retroperitoneal perforation of the duodenum by blunt trauma is primary closure of the perforation with by-pass procedure, whenever feasible.
This report consists of three cases of retroperitoneal perforation of duodenum by blunt trauma, one by auto-accident and two by blow, treated at General Surgery Service of Capital Army Hospital. Brief clinical summary with emphasis on the treatment is presented. One case demonstrated beautifully delineated retroperitoneal free gas in simple abdominal film which prompted establishment of early diagnosis. No mortality or significant complication was encountered.
